Brief summary:
This thesis aims to optimize the management of acute pancreatitis with a focus on patients with a biliary etiology. First we study two severe complication in acute pancreatitis:
- Cholangitis: we study the definition of cholangitis in patients with acute biliary pancreatitis
- Organ failure: we describe the characteristics of organ failure en investigate and the effect on mortality
We then discuss the role of early endoscopic intervention by means of endoscopic retrograde cholangiography in acute biliary pancreatitis. Based on the results of the multicenter, randomized APEC-trial, we conclude that there is no role for early ERCP in patients with acute biliary pancreatitis, without cholangitis. Finally, we focus on preventing recurrent biliary complications after an episode of acute biliary pancreatitis, in which early cholecystectomy plays a central role.
